Please consider joining our Parent -Teacher Association, School Leadership Team, and Parents as Learning Partners. Our Parents as Learning Partners is always the first Friday of every month. On these Fridays, parents can participate in an informal independent reading activity, writing celebration, math activity, and/or a project-based activity. It is a very special time for parents to come into their child’s classroom. All of our children benefit from having more adults to interact with and our whole school community benefits from increased parent involvement. Please mark it on your calendars.

Dismissal Procedures are as follow:
**ALL Parents/Guardians are asked to make eye contact with the teacher during dismissal, so that our teachers can acknowledge that the student is being picked up.
Early Pick-Up: (Prior to 2:00pm)
If you need to pick up your child early, please notify your classroom teacher in writing or call the main office ahead of time. Early pickup must occur prior to 2:00pm. Upon arriving at the school you must go to main office where school staff will call your child. Once your child has come to the main office, you must sign your child out of school. Only a parent/guardian may pick up a child early. You MUST have ID to enter the building and your name must be on the “Blue Emergency Contact Card.” If possible, please make every effort to schedule all medical and dental appointments for after
school hours. If it is necessary for students to leave early during school hours, a note must be sent in and signed by a parent/guardian. Chancellor Regulation A-210
Late Pick –Ups:
It is imperative that students are picked up promptly at dismissal time 2:20pm each afternoon (except those going on to after school enrichment Activities). In the rare occasion when an emergency arises, the parent or guardian must call the school (718) 435-6034 letting us know that he/she is on the way. The student will be escorted to the main office until parent/guardian arrives. The parent/guardian must sign a late pick-up log. You MUST have ID to enter the building.
Absences and Lateness: regarding Minimum Standards for School Attendance Programs. Chancellor’ Regulations require all students to be in attendance at least 90% or more. Students need to be present in order to make satisfactory academic progress. ALL late students must enter through our main entrance on 6th Avenue, receive a late pass from the main office and proceed to their classrooms where they will be marked LATE on the attendance record. Students who are late miss the most important part of the lesson--the purpose and the way to use the skill. It is urgently advised that children arrive at school no later than 8:00 AM. Please remember that report card grades and promotion to the next grade is significantly influenced by attendance and punctuality.
Attendance is an extremely important component of a student's academic excellence. Children should be absent only when they are ill or on a rare emergency. When a child returns to school after an absence, a parent note is required; If it is for medical reasons, (3 or more days), then the note needs to be from a doctor. If possible, please make every effort to schedule all medical and dental appointments for after school hours. If it is necessary for students to arrive late or leave early during school hours, a note must be sent in and signed by a parent or guardian. Communicable diseases such as measles, chicken pox, conjunctivitis (pink eye), pediculosis (head lice), ringworm, etc. must be reported to the school office. Please note that children who are absent or leave school early due to an illness are not permitted to return to school to participate in after school activities or events.
Breakfast in the Classroom (BIC):
PS 94 is a “Breakfast in the Classroom” school that allows us to maximize the availability of free breakfast for all our students. This program ensures that all students receive the proper nutrients and energy they need for their instructional day. Children who have already eaten at home may eat a second meal or not. “Breakfast in the Classroom” takes only about 15 minutes each day at the time the school period starts. Many teachers use the time during breakfast to collect homework and take attendance. Breakfast in the Classroom (BIC) serves the Breakfast Express Menu available at this link. http://www.schoolfoodnyc.org/. Breakfasts include two grain components and fruit every day. Students may also request cereal and milk on any day as well.
Cell Phone Policy: Chancellor's Regulation A-413, Department of Education’s (“DOE”) policy regarding possession and use of: 1) cell phones; 2) laptops, tablets, iPads and other similar computing devices (“computing devices”); and 3) portable music and entertainment systems in schools. Possession of cell phones, computing devices, and portable music and entertainment systems is permitted in schools.
- Students are permitted to bring the following electronic items to school: 1) cell phones; 2) laptops, tablets, iPads and other similar computing devices (“computing devices”); and 3) portable music and entertainment systems (such as iPods, MP3 players, PSP and Nintendo DS).
- Cell phones and portable music and entertainment systems may not be turned on or used during the administration of any school quiz, test or examination.2
- Computing devices may not be turned on or used during the administration of any school quiz, test or examination, except where such use has been explicitly authorized by the school or is contained in an Individualized Education Program or Section 504 Accommodation Plan.
- Cell phones, computing devices and portable music and entertainment systems may not be turned on or used during school fire drills or other emergency preparedness exercises.
- Cell phones, computing devices, and portable music and entertainment systems may not be used in locker rooms or bathrooms.
School-Based Policy of PS 94 for the Use of Cell Phones and Computing Devices on
- Cell phones and computing devices, such as laptops, tablets and IPads and portable music and entertainment systems such as, iPods, MP3 players, PSP, and Nintendo DS may be brought to school but must be kept out of sight at all times.
- Cell phones, computing devices and portable music and entertainment systems may not be turned on or used on school property (inside the gates).
- Cell phones, computing devices and portable music and entertainment systems may not be turned on or used during instructional time including the administration of any quiz, test or NYS examination.
- Cell phones, computing devices and portable music and entertainment systems may not be turned on or used during school fire drills or other emergency preparedness exercises.
- Cell phones, computing devices and portable music and entertainment systems may not be turned on or used in bathrooms.
Students who use cell phones, computing devices and portable music and entertainment systems are in violation of provisions of the Department of Education Discipline Code, the school’s policy and Chancellor’s Regulation A-413 and will be subject to discipline in accordance with the guidance interventions and disciplinary responses set forth in the Discipline Code. These disciplinary measures are listed below:
This notice serves as a warning to the entire school:
- Confiscation of cell phone, computing device, or portable music or entertainment system will be returned to parent during a conference.
Parents should never attempt to call or text message to their child during school day. If you need to contact your child, you should call the General Office at 718-435-6034.
Discipline and Conflict Resolution:
The Chancellor’s Citywide Standards of discipline and intervention measures (Blue Book), which outlines the discipline policies, as set forth by the Department of Education, is distributed at the beginning of every school year to all students. This document includes the DOE’s citywide “Discipline Code” as well as a “Student Bill of Rights.”
All staff members or adults in the building may serve as facilitators for conflicts occurring between students. If a problem cannot be resolved, it is taken to the assistant principal or principal.
In an effort to be clearer to students and parents regarding what constitutes a level grade, we are going to use number grades to inform students and parents where our students stand academically. The following policies are in accordance with NYS and NYC policies.
All K-5 students will receive progress reports and report cards using the New York State levels 1 to 4. Note below the explanation of those levels.
Grades 3 – 5: Yearly grade promotional criteria will be based on three components:
- Classroom work and portfolios
- New York State standardized test scores (a level 2 is required for promotion)
- 90% attendance
We use a grade scale of 100 points that correlates to the New York State four-point grade scale. The section below explains the grade scale conversion:
- 4: (90 -100) exceeds grade level standards
- 3: (80 – 89) meets grade level standards
- 2: (65 -79) approaches grade level standards
- 1: (64 and below) far below grade levels
Classwork/Participation – 25%
Students are expected to engage in their own learning and participate in classroom discussion as outlined in the Speaking & Listening CCLS. The development of oral skills, listening skills and group skills such as interacting and cooperating with peers and teachers is crucial. Classroom participation includes active learning in a group, one on one, peer assessment, and self-assessment. Presentations are assessed with rubrics.
Project Based Learning (PBL) – 25%
In Project Based Learning (PBL), students go through an extended process of inquiry in response to a complex question, problem, or challenge. Rigorous projects help students learn key academic content and practice 21st Century Skills such as collaboration, communication & critical thinking. These projects and products will be used to evaluate how students address community issues, explore careers, interact with adult mentors, use technology, and present their work to audiences. Rubrics used for grading projects/products reflect mastery of common core learning standards
Tests/Quizzes/ Assessments – 20%
Standardized test results from NYSED and NYC as well as teacher created tests, end of unit test and student-generated tests are used for grading students’ mastery of common core learning standards by grade level.
Homework – 20%
Homework assignment is meaningful and purposeful. It extends, reinforces and connects to class work. Homework is collected daily and reviewed for assessing the degree of learning that is taking place.
Attendance – 10%
Attendance is very important for mastery of material. Therefore, absences will affect the quarterly grade. In addition, punctuality is expected from every student. Students are encouraged to develop good habits of attendance, beginning with their first school experience. The attainment of at least 90% attendance is one of the criteria for promotion.
Makeup Work and Incomplete Grades
Students are permitted to make up assignments and tests without grade penalty after an absence.
All students must have a signed permission/consent slip returned to their classroom teacher prior to going to any field trip. If the child does not return the written permission/consent slip they are NOT permitted to attend the trip. Verbal consent will not be accepted on the day of the trip. NO exceptions! Individual teachers will decide the number of chaperones required for a trip. Teachers will do their best to accommodate chaperones and will consider allowing every parent the opportunity a chance throughout the year to attend.
Children are given homework every day. If your child reports that he/she does not have homework, please check with their teacher. Additionally, if you find that homework is taking too long nightly or if the homework does not seem to be at the appropriate level of challenge for your child, please discuss this with your child’s teacher and Assistant Principal.
Lunch & Recess:
Lunch is provided daily. Parents may also send their child to school with a packed meal. Please be mindful, when packing lunches and snacks, to provide your child with healthy choices. The lunch menus can be viewed at www.opt-osfns.org.
- Lunch- Period 3 is at 9:45 – 10:35A.M.: Grade K & 1
- Lunch- Period 4 is at 10:40 – 11:30A.M.: Grades 2 & 4
- Lunch- Period 5 is at 11:35 – 12:25P.M.: Grades 3
- Lunch- Period 6 is at 12:30 – 1:20: Grade 5th
Weather permitting; students generally have an outdoor recess after lunch. In cold or inclement weather, students are offered alternative indoor activities.
Lunchtime/ Recess Volunteering:
We have set up a system of parent volunteers to help facilitate lunch and recess. Parent volunteers are needed daily – one parent per class. Everyone’s help is needed a few times during the school year. Each grade has a parent representative that schedules the volunteers on a monthly basis.
Lost and Found:
Please use an indelible marker to write your child’s name on all outer clothing (book bags, hats, gloves sweaters, boots…) and on lunch boxes. This will enable us to return lost articles. The volume and quality of clothing “found” is staggering. All unclaimed items will be donated to charity after two weeks. Please check the lost-and-found in the Main Office next to the Parent Coordinators’ office.
In the event of an emergency, should you need to get a message to your child during the school day, please call the main office at (718) 435 -6034 and leave a message for his/her teacher. The teacher will relay the message to your child.
Moving or Transferring:
The school and classroom teacher must be notified in advance about family’s plans to move or transfer a student to another school. For more information, please review Chancellor’s regulation A-240 at http://schools.nyc.gov/RulesPolicies/ChancellorsRegulations/default.htm
The Office of pupil Transportation www.opt-osfns.org/opt provides information for families eligible for the school transportation. Please contact the school secretary if you need information or forms on transportation options for your child.
Toys and Electronic Devices:
All forms of toys and electronics devices are prohibited. School personnel will confiscate such items. Parents must claim seized items in person at school. P.S. 94 school staff is not responsible for lost or stolen items.

- S. 94 has adopted, in accordance with Chancellor’s Regulation, a mandatory uniform policy. P.S. 94’s Uniform Policy was decided by our School Leadership Team in consultation with our PTA and parents. The uniform policy is intended to help schools promote a more effective learning climate; foster school unity and pride; improve student performance; foster self-esteem; eliminate label competition; simplify dressing and minimize costs to parents; teach children appropriate dress and decorum in a school environment; and help to improve student conduct and discipline.
The uniform colors are:
Gold button-down shirt with collar and school emblem. NO Tee shirts!
Navy blue pants, Navy blue sweatpants, Navy blue skirt, jumper or skort
Optional: Navy blue sweater, Navy blue knee length shorts
Physical Education: On Gym days only, children must wear navy blue sweatpants, navy blue tee shirt with school emblem and sneakers.
- In very hot weather, the children may wear navy blue knee length shorts with school emblem.
To encourage and reward students for wearing uniforms each day classes with 100% participation for the month will receive a prize or special privilege. Our goal is to improve student performance by eliminating competition in clothing and teach students appropriate dress and decorum in their “work” place, school. Uniforms also help to improve student conduct and discipline, and build self-esteem. This policy can only succeed with the full cooperation of all parents. As per the Citywide Standards of Intervention and Discipline Measures, The Discipline Code and Bill of Student Rights
and Responsibilities, disciplinary response will be taken when students do not follow the mandated policy.
All Students have a yellow vinyl communication folder with a P.S. 94 logo that goes home with them every night and returns to school with them in the morning. The folder will hold important notices for families and may serve as a vehicle to return notes or permission slips to teachers. It is imperative that parents check the folders each night and return them to school daily in their child’s backpack. Parents will be responsible for replacing lost, misplaced, or damaged folders with a substitute yellow vinyl folder.

Parent Teacher Conference:
Parent Teacher Conferences are held four times throughout the school year. There are both a Fall Conferences and a Spring Conferences. The fall conference is held in Sept & November and the spring conference is held in March & May. They are held during afternoon and evening hours. You will receive additional information in your child’s folder. In addition to the conferences listed above, conferences between parents and school personnel are excellent practices that should take place throughout the school year to keep abreast of your Child’s academic progress.
Parent School Contacts:
School Messenger System will send recorded messages to your home phone number or cell numbers regarding school related events/issues. Please notify Nellie Hernandez or Anna Ta, Parent Coordinators should you not be receiving messages. Parent Coordinators will also send important information via e-mail blast. Sign in to see your child's recent test scores, attendance records, grades and contact information in one of ten languages on a computer, phone, or tablet.
To set up an account you will need your child's Student ID number, which you can find on his or her report card, as well as an Account Creation Code, which your school will supply. You can also open an account by visiting your Parent Coordinators – Elaine Ayala Nellie Hernandez or Anna Ta -you will need a photo ID such as a driver's license or passport.
Safety and Wellness
Blue Emergency Contact Card:
Be sure to accurately fill out all the information requested on the Blue Emergency Cards. Please list multiple numbers for emergency contact information. The more phone numbers the better. Any adult that your child can be released to must be listed on the Blue Emergency Card. We WILL NOT release your child to any person not listed on the Blue Emergency Card. Should you have a court order please submit a copy of the court-signed paperwork along with the Blue Card so that we maintain this information on file. If information changes at any time during the year, parents must come into the Main Office and update the Blue Card.
Emergency Closings and Delayed Openings:
The Chancellor of the NYC Department of Education determines school closings and delayed openings by 6:00am. Parents can find updated information by checking the DOE website (http://schools.nyc.gov), calling 311, or listening to local news radio (1010WINS), local TV stations, or cable channel NY1. Any school closings will effectively cancel all field trips, after school programs, and school transportation for that day.
Emergency Evacuation Plan:
In case of an emergency requiring evacuation during the school day, students and staff will evacuate to the following sites: P.S. 1 Bergen School, 309 47th street, Brooklyn NY 11220, and P.S. 169 Sunset Park School, 4305 7th Avenue, Brooklyn 11232.
Fire Drills at School (Evacuation Drills):
Fire drills are held throughout the school year to prepare students and staff for an emergency situation. During a fire drill it is an expectation that all students remain quiet and follow the directives of the teacher to quickly and quietly exit the building. Please stress to your child (ren) how important these rules are and review them often at home.
All students entering a New York City school for the first time must have a complete physical examination. The comprehensive examination must include proof of all required immunizations. Information about these requirements is available on the NYCDOE website https://schools.nyc.gov/office/health
Notify the school nurse of any chronic conditions diagnosed in your child. Please return all physical exam forms to your child’s teacher.
Lockdown Drills at School:
Lockdown drills are held throughout the school year as part of the General Response Protocol (GRP). These drills help to prepare students and staff for a threat within the school building. During a lockdown drill all students and staff are required to be in a locked location and remain quiet and out of the,” line of vision”. Advanced notice of these drills is given to parents so that you can speak with your child at home and stress the importance of these drills. Your child’s safety is of utmost importance to us at P.S. 94.
Media Release Form
“Consent to Photograph, Film or Videotape a Student for Non-Profit Use” permission slips are distributed school-wide in September. These permission slips are collected and secured by our Administrative staff. With this form the parent gives permission for child to be interviewed, photographed and videotaped by the Department of Education as well as consent to be used for non-profit purposes including use in print, on the internet and all other forms of media.
Nurse’s Office and Medication:
Mrs. Faina Brenkler our full-time school nurse is located in Room 106. If a child is ill or injured during school hours, he or she will be taken to the nurse’s office. Should your child required a 504 form for medical accommodations, (use of asthma pump, dispensing medication, Epi- pens or monitoring of sugar level) a parent must complete and submit a Medication Administration 504 form, available from the school nurse. To reach the nurse, you may call 718 435-6034 ext.1041. For your child’s safety, in case of a health emergency and no one can be reached, the Principal will call 911.
School Safety and Security:
All visitors & volunteers to the school must sign in and show photo identification at the security desk in the lobby to receive their visitor's pass. Visitors must then go to the Main Office and state the nature of business to one of the secretaries. When exiting the building please exit through the main entrance on 6th Avenue. It is essential for the safety of the children that we follow this protocol.
Shelter Drills at School:
Shelter drills are also held throughout the school year as part of the GRP. These drills are to prepare students and staff for an emergency or threat outside of the school building. During these drills the front door of the school is locked and no one is permitted to enter or exit the building. During these drills all business throughout the building is conducted as usual.
Response to Intervention (RTI): Academic Intervention Services for At-Risk Students
Our work with at-risk students is being supported by regulations which require all schools in New York State to provide academic intervention services for students below the State designated performance level on State assessments and/or who are at-risk of not achieving the State learning standards.
RTI services are not special education services, but are additional instruction and support services, which supplement the general curriculum, before referring a student for special education services.
In following the Response to Intervention Act, there are three tiers of intervention:
Tier 1 - support that teachers provide their students in the classroom,
Tier 2 – push-in/ pull-out support services by an AIS teacher with a small group of students
Tier 3 - more intensive than Tier 1 and Tier 2 services, could be one-on-one, with an AIS teacher.

- Literacy Prevention and Intervention After-School Program: Grades 1-3
- Intervention Mathematics: Grades 1-4
- Saturday Academy: Grades 3-5

One of our top priorities as a school is to meet the needs of all students as we strive to meet high standards of performance.
Dual Language (Spanish or Chinese)
As a school, we believe that a dual language program will help our students to become bi-literate, bilingual and bicultural. Children who know more than one language have intellectual, educational, personal, social, and economic advantages that will continue throughout their lives. We follow a Balanced Language Model (50/50 model), reflecting the fact that the two languages are evenly balanced. English language learners (Ells) receive half of their content instruction in the new language (English) and the other half of their instruction in the home language (Spanish or Chinese). This is considered a “simultaneous literacy” model as both the new language and the home language are focused on simultaneously. Our content areas are closely aligned to the language of instruction of the lesson and lessons are not translated to both languages as the skills are not retaught in both languages, but rather build upon. The students are linguistically integrated; classes may contain students whom are native speakers of English, native speakers of other languages or multilingual (students who have reached proficiency levels in more than one language). Upon their completion in the Dual Language programs students in both language groups will:
Meet or exceed Next Generation Learning Standards
Develop proficiency in their home language
Develop proficiency in their new language
Develop positive cross-cultural attitudes; behaviors, and skills that will help them function in a global society
Develop second language skills while learning content knowledge in both languages.
English Language Learners (ELLs) `
We provide ongoing support and guidance in the effective implementation of our Dual Language and ENL programs. These programs strengthen students’ home language development and content knowledge while they build their social and academic English skills. We also provide English as a New Language (ENL) programs that use strategies for English language development with home language support so that students develop language and content knowledge in English.
Upon admission, parents complete the Parent/Guardian Home Language Identification Survey; this survey will be used along with other information provided to determine your child’s home language and eligibility for the New York State Identification Test for English Language Learners. Once child takes the test, instructional planning and delivery of English language support is identified.
Gifted & Talented
Gifted & Talented (G&T) programs are one way that the NYC Department of Education supports the needs of exceptional students. G&T programs aim to deliver accelerated, rigorous, and specialized instruction aligned to Common Core Learning Standards (CCLS). Students entering kindergarten through third grade are eligible to participate in G&T admissions. If you believe your child requires additional academic support, please contact an Assistant Principal to address your child’s needs. The school has a team of experts that will differentiate instruction and create plan to support your child’s needs. In addition, we have a pupil personnel team composed by a school psychologist, social worker, and a special education teacher that can support you with the process of conducting an academic evaluation to determine the best learning setting for your child, which can include SETTS, a special education class or an Integrated Classroom Team setting.

Parent Associations are the backbone of the public schools. They provide the parent leadership essential to developing school strengths, identifying and solving problems and building the partnership between schools, community and parents necessary to successful education. The New York City Board of Education, recognizing the importance of parent associations to a school’s success, issues this policy statement to foster strong and effective parent representation in every school and at every level of the school system.
Parents as Learning Partners
8:10 a.m. - 8:55 am
Please join us on the first Friday of every month for our “Parents as Learning Partners” program. The first Friday of every month is a very special time for parents to come into classroom, and so please mark it on your calendars. On these Fridays, parents can participate in an informal independent reading activity, writing celebration, math activity, and/or a project based activity through social studies or science.
All of our children will benefit from having more adults to interact with and our whole school community benefits from increased parent involvement. Thank you.
